For form six applicants

Holders of Ordinary Diploma or Equivalent Qualifications

APPLICATION   REQUIREMENTS

Photocopies of the following

  1. One certified photocopy of both ‘‘O’’ and ‘‘A’’ level pass slips or their Equivalent
  2. One photocopy of the receipt of the application form
  3. One photocopy of the ID One recent passport-size photograph
  4. One photocopy of the birth certificate
